How to schedule a well-child appointment/physical exam
- Call your CPCMG pediatrician’s office between 8am – 5pm, Monday through Friday to schedule an appointment. Not sure about the office phone number? Visit our Locations page.
- You can also use your MyChart account to schedule appointments. If you don’t have a MyChart account, learn more here.
- Please schedule these appointments four to six weeks in advance so that we can better accommodate your choice of day and time.
- If you’re a new patient, please call the CPCMG Welcome Center at 858-502-1177 between 8am and 5pm, Monday through Friday. They will register your child, confirm your insurance information and schedule your first appointment.
How to schedule a sick-child appointment

- You can also use your MyChart account to schedule appointment. If you don’t have a MyChart account, learn more here.
- For health advice for your sick child between 8am and 5pm Monday through Friday, you can call Nurse Care Connect to speak with a nurse and have your health care concerns and questions answered. Simply call your CPCMG office and listen for the option “Health Care Advice” – you will then be transferred directly to a nurse.

Preparing for a well exam, school/camp/sports physicals and immunizations:
- If you have a MyChart account, please complete the Well Child Questionnaire prior to your appointment.
- If you need us to complete any forms (such as school, sports or camp physicals), please call ahead and check with your CPCMG office to see if your child needs an exam before we can complete the forms.
- Please bring any forms that need to be filled out with you to the appointment.
- It is extremely important that you bring in your child’s immunization card. Please note: There is a charge to replace a lost immunization card.
- Arrive 20 minutes prior to scheduled appointment time.
- Please note there are medical forms that need to be completed for your child’s well visit that can be found on MyChart. If you do not have a MyChart account, you will need to fill out these forms in the office.
Helpful tips for a successful appointment:
- Be sure to bring your child’s current health insurance identification card and co-payment, if applicable.
- California law requires that your child be immunized before enrollment in public or private schools. Please discuss your child’s immunization schedule with your child’s physician.
- We would appreciate your help in not bringing food into our offices.
- Please refrain from using cell phones while in the exam room.
New patient?
New patients should download and complete the New Patient Packet (paquete de informacion de paciente nuevo). Bring your completed packet to your first appointment and plan to arrive 15 minutes early.
Patients who have not completed the New Patient Packet should arrive to their first appointment 30 minutes early to have time to complete the packet.

If you’re running late for your appointment
CPCMG will do our best to accommodate patients who arrive late for their scheduled appointment time. However, you may be asked to reschedule your appointment for another time if you are more than 15 minutes late.
How to cancel an appointment
We ask that you call our office 24 hours in advance (or as early as possible) if you need to reschedule or cancel an appointment.
“No Shows”
A “no show” is a patient who fails to arrive for a scheduled appointment without cancelling at least 24 hours in advance. CPCMG staff will follow-up with all no shows, to ensure that follow-up is provided as deemed necessary by your provider. Patients with three consecutive no shows in a 12-month period may no longer be able to receive services from CPCMG.
Appointments where you cannot accompany your child
After your child is an established patient with CPCMG and you are unable to bring him or her to their appointment, you must send a note signed and dated by you with the following information:
- The name of the person accompanying your child.
- Permission to see and treat your child in your absence.
- Permission for any immunizations that your doctor may recommend.
If you have identified an adult who will regularly accompany your child to their medical appointments, you may complete a third party consent form (access the Spanish version here).
